NHG has been participating in the four-year, EU-funded I-CARE4OLD project, which aims to improve prognoses and assess the impact of treatments for older care recipients with complex chronic conditions in home care and nursing home settings. The project also seeks to develop, validate, and test next-generation individualized decision support tools.

Over the past few years, NHG, in collaboration with other project partners, has been developing machine learning models to assist clinicians in managing the most complex cases with enhanced quality and predictability. Meanwhile, the project’s software partner has been working to ensure their platform can process these models and serve as a practical tool for clinicians. Now, it’s time to bring all these individual components together!

Last week, NHG’s researchers, Anna-Maria Hiltunen and Dr. Mikko Nuutinen, presented their poster at the EuGMS 2024 conference in Valencia. They shared preliminary results from the tool’s pre-pilot phase as well as insights gained from stakeholder engagement.

The pre-pilot phase has already provided valuable insights into the tool’s potential to support clinicians in making more informed decisions for older adults with complex care needs. Stakeholder engagement has been crucial in refining the tool to ensure it meets the practical needs of healthcare professionals working in diverse settings.

Looking ahead, we will focus on expanding the pilot phase accross multiple sites, gathering more comprehensive data, and continuing to refine the tool based on user feedback. Our vision is not only to enhance the decision support tool but to pave the way for a new era of data-driven, personalized care in home care and nursing home settings. By integrating advanced machine learning models, we aim to empower clinicians with the ability to anticipate outcomes more accurately, tailor treatments to individual needs, and ultimately improve the quality of life for the most vulnerable patient populations.

In the longer term, we hope this project will inspire broader adoption of predictive analytics in geriatric care, revolutionizing how we approach complex chronic conditions on a global scale. We envision a future where clinicians have the tools to provide proactive, personalized care, leading to better health outcomes and more efficient healthcare systems to everyone.
